We will be in contact with the client within 48 hours to organise an initial assessment.

Once the inital assessment is arranged, you will receive an email from us with confirmation of date of initial assessment.

We pride ourselves in making the referral process easy for our referrers, this is how it works

You complete the referral form and email it to "referrals@chidleycarephysio.com.au" or complete the online referral form above.
We receive the referral form and contact the client within 48 hours to arrange an initial assessment.
Once the initial assessment has been arranged, we contact you as the referrer to confirm the appointment to ensure you remain updated.
We attend the clients home and complete the initial assessment.
We send you a comprehensive report from the initial assessment, with an easy to read summary of the assessment findings, outcome measures and with our recommendations of treatment frequency.
We remain in contact with you, bringing any changes to your attention.

Frequently asked question:

Do you need a referral to see one of our in-home physiotherapists?
- Most likely not, in South Australia you do not need a referral to see a physiotherapist most of the time. If you are a private client looking for a physiotherapy appointment, give us a call on (08) 8472 8296 and one of our friendly staff will be able to book you an appointment. If you are a referrer (HCP manager, GP, etc) it is preferable if you complete the referral form above.
- If you are and NDIS or HCP client, give us a call on (08) 8472 8296 and provide us with the contact details of your care coordinator so we can arrange an appointment for you
- If you are a Department of Veterans affairs (DVA) or Enhanced Primary care plan (EPC) client, you will require a referral from your GP. We are more than happy to provide you with guidance for this process, speak to us on (08) 8472 8296.
